How do I set a conditional field based on the response to one or more radio button groups?

Overview

When you are working with forms, it’s a common requirement to set a conditional field based on the response to one or more radio button groups (OR Logic).

Objective

The main objective of this document is to educate a DocuSign administrator to create a form with multiple radio button groups and a text field that is conditional based on the responses to one or more of the radio button groups.

  1. Add radio buttons to the template
Add all radio buttons to the document. Each question should have its own group of radio buttons.

  1. Create text fields to each radio button
For each radio button group, we need to add a text field.

Each text field should have the following properties:
  • Required Field: Checked
  • Read Only: Checked
  • Add Text: 1
  • Character Limit: 1
  • Color: Same color of the document (in this case, white)
  • Validation: Numbers
  • Conditional Logic: This field should appear only when the related radio button is selected.
 
  1. Add a formula field 
After adding all text fields, we should add to the template a formula field with the following configuration:
  • Formula: Sum of all text fields >=1, for this example [Q1] + [Q2] + [Q3] >=1
  • Decimal Places: 0
  • Color: Same color of the document (in this case, white)
*Special notes about the formula field*
Using the mathematical sign of greater than or equal to (>=) in the formula will result in a comparison/evaluation of the result, NOT the actual total of the mathematical equation. In this example, even if all 3 text fields with ‘1’ in them were activated by the radio button selections, the formula field will only display ‘1’ as we set the formula to compare the result to 1, NOT to total the result. When defining comparisons:
1 = true
0 = false
 
  1. Add text field for conditional question
Add a text field to the template for the conditional response to the radio buttons.

The text field should have the following properties:
  • Required Field: Checked
  • Read Only: Unchecked
  • Conditional Logic: This field should appear only when the related formula field is = 1

Now you’re ready to send envelopes with multiple radio button groups and a conditional text field based on the responses.
